Community Care Program
The Community Care Program is offered through the Illinois Department on Aging. This program is designed to assist you in remaining more independent in your own home , and to keep you out of a nursing home, as long as nursing home placement is not necessary for you.
The Case Coordination Unit will make a home visit to discuss your needs and to determine whether you qualify for the Community Care Program include Homemaker and Adult day Care. The Case Manager will complete all the necessary paperwork for you to determine eligibility.
Homemaker: Homemakers are available to dust, vacuum, clean the kitchen and bathroom, do laundry or prepare meals for older persons who are able to perform only some basic household tasks themselves. Homemakers also assist with personal care, such as grooming and bathing, for older persons who are moderately impaired. Assistance is also available for more complex tasks, such as meal planning and household supervision, for older persons who have difficulty managing these activities.
Adult Day Services: Adult day services offer older persons the opportunity to interact with other people in a supervised setting outside their home. These services may include social activities, noon meal, participants may enjoy everything from arts and crafts to card games and current event classes. Transportation is also provided .
You are eligible for the community Care Program services if:
- You are age 60 or older;
- You are determined physically in need of service, meaning you are at least moderately impaired;
- You are a resident of the state of Illinois
- You are a United States citizen or legal alien; and
- You meet the asset requirements, which will be explained to you when the Case Manager comes to your home.
Cost:
If you meet the eligibility requirements and if your family income is below the federal property level, the State will pay the entire cost of service; you will not be charged a fee.
If your family income is above the federal property level, the State may still pay part of the cost of service.
Choices for Care Assessments
The Case Coordination Unit will complete an assessment for anyone who is entering the nursing home. This program is designed to help families discover their options for long term care.
Making decisions about long term care can be overwhelming for you and your loved ones. Through the Choices for care Program, a trained professional can provide a helping hand at no cost to you. Case Managers in your community will provide a free consultation that includes an assessment of your needs, a review of service options and costs, and assistance in developing your plan of care. The Case Manager can help you determine which services are appropriate , available and affordable to you.
The Case Manager will ask you questions about your ability to perform certain tasks, such as cleaning, shopping, preparing meals and managing medications. Your ability to get around your home will also be discussed. This information will help the case manager identify your needs. You can then choose the appropriate and affordable service. The case Manager will come to the hospital or to your home, before placement is planned into a nursing home.
