Our Agency provides in-home assessments to seniors 60 years of age or older. We provide Case Management Assessments and also complete choices for Care Assessments for anyone who enters he nursing home. The West Central Illinois Case Coordination Unit is the Elder Abuse Provider agency for our Area. The agency also administers the Community Care Program. The Case Coordination Unit covers the following counties: Adams, Brown, Calhoun, Hancock, Schuyler and Pike in Illinois. There is a Case Coordination Unit for every County in the State of Illinois.

Case Management is a service to help you stay in your own home. A professional Case Manager will arrange for the services that you need. These may include assistance with housekeeping, laundry, shopping, personal care, errands, meal preparation, financial assistance, transportation, housing rehabilitation, housing relocation assistance and a large array of other services. Only you and your case manager will know.
